PROPERTY OWNERS

PLEASE FILL OUT THIS PERMISSION TO CONTROL CLP FORM

The MN DNR requires each property owner who elects to be included in the curlyleaf pondweed control project to fill out this permission form to control (spray with chemical) within 150 feet of his or her property.  Please fill it out and return it (hopefully also with your pledge or donation) .

 Do this even if you do not plan to make a donation at this time.

If you do not complete and return this form, your property will not be sprayed, leaving you with the full responsibility of controlling this invasive on your shoreline personally.  Not treating will give curlyleaf a better chance to spread and establish around our lakes making it ever harder and more costly to control.  Help the lake and your fellow lake owners by participating in this project and sending this Permission to Control in promptly.  Don’t allow your lakefront to become a CLP seed bed!

HOW TO FILL THE PERMISSION TO CONTROL FORM:

Per the example below, please fill in all of the regular fields and include both address areas if that applies.  Having your phone number, cell number, local summer address, and e-mail address will make it easier to contact you regarding the progress and any issues concerning this project.  All information given will be kept confidential and only used by the DNR or CLA to inform you of environmental issues of interest to Cullen Lakes Property Owners. 

 In the area labeled “SIZE OF AREA PROPOSED TO BE TREATED”: If you know the amount of shoreline you have you can fill that in, but the rest is not necessary as the DNR has access to that information from the county records.
